Online surveys

Hi, I'm looking to do online surveys mainly based on the employees feedback of managers. Ie. The employee goes on the site and fills out a survey.
Could I get some general direction as to 1) What are some good software packages, 2) What are good languages for this, and 3)What are some recommended database/report handling techniques for this.
