One-X app VOIP no audio when on 4G

Hello all,
I'm using One-X Portal on R9. I'm using the One-X Mobile for Preferred IPO app on my iPhone 5c.
The problem is no audio on a VOIP call when I'm outside my LAN, not even ringing on the app.
I have a second problem that when I call an extension while on 3G, I hear ringing on my iPhone but when the extension picks up their end, the app doesn't register that the call has started and says "call failed".

I can connect properly, and even make a successful call when on local wifi.
I can connect properly on 4G but no audio on a call.

Ports forwarded:
5060 (TCP/UDP), 5061 (TCP,UDP), 49152-53246 (UDP) are forwarded to my IPO
5222,5269,8063,8080,8444 (TCP) are forwarded to OneX (Linux application server)

I have DNS setup properly internally and externally.
SIP remote extn enable is enabled.
RTP port range matches the forwarded ports.

Any thoughts?
