one extension queued to another extension

I managed to find a previous thread about this issue, but cannot resolve it.

A user (ext 445) has both SA Voice and SA Ring lights flashing on her phone.
She cannot make calls, and cannot receive calls.

If she pushes either button, it announces it is queued to another extension.

I have checked and the SA-Orig button is still programmed, as are SA-Voice and SA-Ring.

I went into centralized programming and prodded around, but couldn't find anything.

after doing so, this user reported that the condition remedied itself.

However, around the same time that I did that, she said the extension she had been queued to, called and she hung up.

so we are not certain which action corrected the problem.

She did say that it happens on occasion, but this is the first time she has reported it.
Also it's usually not the same number.

and one last thing, she was not in her office at all for the entire morning. she just came in and it was queued.


Hope this helps someone else, or helps figure something out.

how would I know if lines were "dead"?
 
loop start would be an analog line correct? I don't have any of those connected to the system.
 
She is the only person to ever report this event, so I'm assuming it's just her phone.
The phone is a 408MLX. I believe the typical 3 SA buttons are still programmed as such:

SA Orig
SA Voice
SA Ring

(or at least that's what we've always typically had since I've been here).

 
no I have not seen it fail.
she says it has happened on occasion, but only reported it this one time.

and no, it's a MLX16 :)
