One desktop to other through internet using VNC

Hello everybody
I am using VNC to send one desktop to other. I want to modify such that i can send desktop on internet. I am getting problem if i install that software on server where i have put my sight then it is not possible to send on desktop to other bcoz it will capture server desktop. How can i get the IP of both system so that i can send one desktop to other through my existing sight.

one more problem if the system exist on proxy.
