Old Hard Drive?

I've got a hard drive that is getting very noisy, and spending a lot of time spinning around when the comp is not being used (Win98). I've eliminated temp files, defragged, formatted (which helps for a short time) and eliminated possible viruses. Is this a hard drive that is just worn out? How can I find out what it's searching for? I know more RAM might help, but...
 
If it's close to full, and you don't have that much RAM, the swap file is being used extensively. Add more RAM, and/or a bigger (newer/faster) HD, and the problem will go away. Cheers,
