I have a user who had two old (previously unseen) emails show up as new yesterday - one that was sent last April and one that was sent last week on Monday. She says they never arrived in her inbox previously. They are internal messages (same office & domain). Nobody else has reported similar problems. She has no Outlook rules/agents set up. We have an Exchange 2003 server and Outlook 2003 on the workstations. Anyone else seen and resolved such an issue? Thanks in advance!

You probably restarted your server, and some old, overlooked files that hadn't been deleted from the SMTP local queue folder (on disk) got resubmitted to the queue. I wouldn't expect it to happen again, or if it does, with any regularity.
