Odd SIP Trunk issue

I have SIP trunks installed at a site. Beginning this morning, users calling from inside the organization to a mobile (or 911), hear 1 ring, then the call goes to busy. It rings long enough though to display caller ID (we confirmed this with 911 who called us back). All inbound calling works, all outbound toll free, long distance calls work. CUCM 10.5(2) - there is only 1 region.

Any ideas?
 
In chatting with the service provider, they said some calls are failing authentication. A BYE message is sent form our gateway. How can some calls fail and not others? That makes no sense. To add to the mystery, a 3 octet source IP is being created (like a ghost) call along with some calls and not others :|.
 
I run into this from time to time with turn ups. Its a telco issue.

Yes, after calling the telco multiple times - they admitted it was their issue!
