Odd response to disk failure

NW 6.5 sp4b on Dell PE2650. 1 disk of a RAID1 array failed, hardware worked like it's supposed to, no sweat.

I'm a bit puzzled by Netware's response to the event. DS was unloaded, but no sign of the event was logged anywhere and I can't tell exactly when the problem occured. Console was active and I was able to do an orderly shutdown. Rebooted clean.

Why would DS unload? And why would it unload without logging the fact?
