NT4 user account lockouts

I have a question that maybe someone can answer. We're experiencing problems where users are logging in to the PDC/BDC and getting locked out after the first login. We have our policies set to lockout after the 3rd attempt. Any ideas why this is occuring?

Check your Event Viewer, Security Tab.
Maybe you're getting hit from the outside.

Had they happen before.
