What changes were done to the system between when it did work and when this problem came up?

Unfortunately I'm not sure, I didn't get involved in the problem until quite late. I think there was a lot of swapping hardware around, disks, raid hardware etc. I've created a new pool and volume from spare space on the RAID array and restored from backup, so we've worked around the problem.
