Notes/Attachment Issues

Hello,
I am using ACT! 2006, version 8.0.2.82 and have been running into problems adding Notes and Histories to any Contact.
Once in the Contact Detail, I select the Notes section and then right-click to "Insert Note". Once I select Insert Note, the program freezes. I am able to use other programs (IE, Outlook, etc.), but have to use Task Manager to close ACT! and restart the application.
This also happens when I try to Record History on the History tab or create an Opportunity for the contact.
I can add Activities to the contact, so I'm not sure why it is happening for these other items.

Any suggestions?

I have tried the DB Maintenance, but not the ACTDiag repair. I'm not familiar with that. How is it completed?
