Note / Information box with date and time stamp for each entery

Hello all
Is it possible to create a note box or lob box so that an entry on a form is date and time stamped every time there is an addition to the record? I am tracking dial tone at different locations and would like to add notes to the record every time a change is made starting with the installation of the circuit.
All suggestions are welcome

In other data bases - Oracle, SQL Server - there are Triggers. In Access you have something called Data Macros. Certain code/actions may be automated when record gets inserted, updated, etc. That's where you can say: when this happen, update this field to Now()

Can you give us some information regarding your table(s)? Are you updating a single long text field in a single record or adding a record with each note?
