Our web developer designed a page that will allow you to submit information for a sales person to contact you back, but once you hit the submit button the page wants you to authenticate with a windows account and if you hit cancel you get the following

You do not have permission to view this page using your current user account.
Please try the following:
If you have another user account with a higher level of permission, click
your browser's Back button to try again using that account.
If you believe you should be able to view this page, contact the Web site administrator.
I've removed Windows integrated check box, assigned Read Write to Anonymous and full rights to Network Services. The server is running WSS 3.0
