Nortel CICS no ring or appear on sets

I have a Nortel CICS running 4.1 with a 4-line VM card. Suddenly incoming calls are not ringing or flashing on the sets. Ring & Appear is on. We can call out and can answer incoming calls. They are just not flashing or ringing. There is one line out of 3 that does still ring. Any ideas what may be the cause?
 
Maybe line issues from the street like not enough voltage etc.

First thing a tech does is check the line before the KSU with a test set.
The second thing to do is swap the good line with a bad line, this will determine if its a bad Line Card (not VM card)

Can you answer it even though its not ringing or flashing by just pressing the line button?

Yes it was actually the CID expansion card I was thinking of, not VM. We can answer by just picking up or by choosing the line button.

I'll try swapping the lines. One thing I noticed is the good line rings on the wrong assigned line number. It's assigned Line 3 and rings on line 2. Don't know if that was always the case. I inherited this system not long ago.
 
When you go into Lines in programming note the actual line number and the name.
i.e. LINE 003 name Line 2

This indicates a previous issue such as Line 2 went dead so the dialtone was moved to LINE 003 and renamed to Line 2, in this case I usually rename LINE 002 as DEAD

After swapping line 2 with line 3 at the 66 block, the good number still rings. Now it's at Line 3 of the KSU. I think that hints at issues from the provider.
 
Yes, in this case I am then going with not enough voltage from the carrier.
