Norstar 8x24 resetting when conectting a new station

Hi, I've got a customer with a Norstar 8x24 Rls 4 with a 6 port copper exp. card that has connected two 12x0 modules and three 0x16 digital extension modules, When users from Two digital extension ports try to call external or internal the PBX is resetted, I move those Two digital extension that caused the failure to another ports solving the problem. But now the customer wants to connect 7 new digital extension to users, I connected digital extension to 3 users, but when I connected the fourth digital extension all the stations and PBX resetted, I moved that digital extension to several ports with the same result, I also tried with diferents digital telephones sets. I tried connected the fourth digital station directy in the PBX MDF. The PBX has connected about 55 digital extensions connected to users from the total digital extensions (72).
Please any ideas to solve the problem?
 
Check the wiring of those ports at the MDF, you may find you have crossed pairs.

If the wiring checks out then you might have corruption or a bad software cartridge.

What might fix corruption:
-Doing an upgrade (to DR5 in your case) - power down, swap software cartridge, power up an wait.
-Perform a STARTUP (Factory Default) and re-program all from scratch can sometimes clear the glitches too

Maybe's:
-The 6 Port Expansion is causing havoc but when the issue is on the main cabinet too it then seems unlikely but you never know.

-Look at the release & version number under the phone and see if the bad ones are all the same.

-You mentioned several times that you moved sets to other ports but not once did you say if you tried ports on a different 0X16 module or the main cabinet.
So if you have only tried different ports on your one 0X16 then try the others and main cabinet as this will eliminate it being a module issue or it's cable.

"I also tried with different digital telephones sets." You did not mention if the other sets you tried were spares or existing: I tried with diferent sets models:M7100, T7100 and M7208 and all caused the PBX reset

I'm going to try to unplug an operative digital set and then connect a new free digitsl port, It could be a power supply overload? but I tried different ports on the three 0X16 and main cabinet?
