Non-existent .MSI file

Does anyone know how to get rid of a non-existent .MSI message screen whenever I do anything in Windows? This .MSI file is nowhere to be found, the in-house developer said it does not exist. (In-house developer created app)

It's very annoying since you have to click cancel several times before any application will open. Some will just hang and I have to end task praying that it doesn't crash the PC.

Also, Add/Remove programs has nothing in it so I can't just simply remove the app. Some COM components were registered/setup manually. I have reinstalled IE 6 and MDAC. Don't know what else to do. Before reinstall of IE 6 Windows Explorer did not show the Folders, but after it did. Very strange indeed.
