No internal dial tone

The interanl ringing you hear when calling another ext stops working, reboot and it comes back for a while , maybe hours or if your lucky days. You can call someone and the call progresses as it should except you dont here any ringing in your earpiece

My gut = faulty handset or module, I would be reflashing that firmware and starting agian.

From there call AVAYA !!!

TDM or IP Phones? If IP double check your codec and make sure the IP Handset you're using supports it. You will experience almost exactly what you're describing if you pick something like ADPCM32K which the control unit supports but 46xx phones do not.

No ip phones, happens on all phones, DT and analogue at same time.They are the older Index type DT phones though

Selecting off hook operation will stop the engaged tone being heard by the caller but should not block ringtone.
It would still be worth checking that this has not been enabled by phonemanager
failing that I would also suggest a firmware update/replacement unit
