I have seen this before. Are you using the 5610 perhaps?
Look at the RJ45 connector in the Phone. From my 140+ 5610 there were 3 where the pins were a bit out of place.
Using a small screwdriver it was fixed. It looked like the supplied grey patchkabels are prone to do this.
