No dial tone on Avaya 9508's 2

I have an IP office 500 V2 8.1 (95) preferred edition , It has two Combo cards and one DS8 with PRI as a daughter card.
When I try to install 4th Card (DS8), all the phones lose their dial tone and if a person calls in and we pick the call it gives them a really loud static .Everything else seems normal.

Things i have already done:

[li]Changing New DS8 card with another that i know work for sure.[/li]
[li]Changed DS8/PRI card.[/li]
[li]Changed Power supply on Chasy.[/li]
[li]Swapped new chasy.[/li]
[/ul]
Any suggestions on what might've caused it or anything i can try to fix this issue would be greatly appreciated!

Remove all cards and put the two DS8 cards in the first two slots.
Then boot the system and see what happens.
If that works then add the two combocards and do the same.
If it fails at any point then remove one of the cards and try again.

It works fine with any 3 Cards, but as soon as we put Card in, all the phones lose their dial tone.
Any idea why'd this happen?

Did you do what i suggested?
Put the DS8's in slot 1 and 2 and then the combocards.

Moving the combo cards to slot 3 & 4 fixed the issue. Thankyou Sir..
