When I send niinp:name=aaaaa; I have a list of 5 extensions with the same name. And when I send sudip:dir=ext; I have ext does not exit.
How to remove old from database ?

You have entrys in you name data base in PBX.
So i would use
NIINE :NAME="Smith John",NUM=2773;
ore
NIINE :NAME="Smith John",TYPNUM=ex;
