New Compaq Drive (MTWEOF) drive index 0 I/O error

I have just installed a new Drive on a Compaq tape library TLD891, each time I try a backup to tape I get bptm error 84 and a message from my job details screen that says ioctl (MTWEOF) drive index 0 I/O error, writing header block I/O error warning downing drive index zero

Here is my bptm log please anybody help.

Library is connected by SCSI cable to Master/Media NB 4.5 server
 
Was this a drive replacement? Is the SCSI ID of this drive in conflict with any other on the same SCSI Bus?
 
I believe that I checked this earlier but it is possible that another device was added to the environment and is in conflict. I will check it out tomorrow and let you know what I find.

Also, if the drive was replaced, the serial numbers may not match and this will also cause the error. When replacing a drive, you have to delete the device from the admin console and add the new device back in.
 
PGPhantom I have removed and re-added drive with Admin console and vmadm, in conjunction with analyzing the drive; no errors found and no conflicts.

Comtec17 I have mapped out all scsi ids for the environment and am in the process of analyzing. I have already changed one pci scsi card's suspect initiator id from default 7 to 6 and will test. Because I am out of the office for a couple of days I will try and respond by thurs let you know what happens...if you guys get any other ideas before then please let me know. h
