NetBackup V5.5, Dell Powervault - drives down

We are experiencing problems with tape drives going down and then freezing the tapes. We have rotated our tapes out and using new tapes to eliminate the possibility that the problem was being caused by bad or dirty tapes. Has anyone else experienced this or found a resolution to this problem. We are using SDLT tapes, running NetBackup v5.5 on a Dell Powervault 136T.

most probable cause....drives are not correctly mapped.
