need v.90 ext. modem power supply specs

Does anyone have the specs for the power supply for a USR external 56k modem? My modem is model FB 0710-D, part # 005686-03. I need volts, amps, plug polarity.

I can get that for you if you can help me find out the plug polarity . It is difficult to find this PS, but other non-USR external modems use a very similar one.
