need to output form to word template?

I need to send (email) a report (an invoice) that is exported from Access (so I guess it's an rtf) - anyway I need the person who receives this report to check a radio button (or toggle - I don’t care) to mark it as approved or denied. Then they send it back to me. I would like to then import this back into Access, read the invoice number and response (approve/deny) and update a table accordingly. Any suggestions? I know that my recipients all have Office 2k or better.

Why not have the users open your database to a pretty form that displays the info, and have them check the box right there in the database?
 
because the report is used to send invoices from corporate to the field to approve the invoices (the report is an invoice) - the database imports raw data, organizes and creates the invoice data - but the invoice needs to be approved. Also - due to the physical locations, and lack of a corporate network - the only way to (electronically) communicate is email.
