Need mailing list data entry program in ADO

I am needing a business contact mailing list data entry program written in ActiveX Data Objects (ADO).

The access data base fields are:

PHONE (primary key)
BUS_NAME
OWNER_MGR
ADDRESS
CITY_STATE
ZIP_CD
CATEG (category code)
UPDA (update code)
CNTC (contact code)
PHONE_2
PHONE_3
LASTUPDATE

Where can I find one?
