Need help with Table/chart design (for people with earing problems)

Hi all!
I have this paper chart, for examining earring trouble(called Audiogram),That I don’t know how to put It’s details in a table – which part should be fields and which should be details. Also after the user put in the details I don’t know how to convert it to a chart.
I have tried using the wizard chart but I didn’t get the results I wanted.
I tried all kind of ways .. maybe I don’t understand how it works L.
The chart looks like this:
It’s a (X,Y) graph – X axis is frequency(in Hz) and Y is volume (in db).
There is a line fore the results of each ear.
The labels for X axis are: 125 – 250Hz, 250 – 500Hz,500-750Hz,750-1000Hz … ext.
The Results should be something like this(with lines between points)

600 L L
400 R
200 R
100 L R
125-250 250 –500 500-750 750-1000

I used to do a lot of fancy charting with earlier versions of Visual BASIC and 3rd party products. I'm not sure the graphs built into Access can do what you want. I can think of two options:

1. Create a report to do this with custom VBA code. Using a Courier-type font, you can get things to line up, but probably not connect the dots/letters like you want.

2. Look into third party graph products (OCX, etc.). You might try the Programmer's Paradise website or catalog. It may take the VB product to do what you want.
