Need help with creating a dynamic questionnaire

I need some help with creating a dynamic questionnaire in Live Design from Adobe Acrobat 8.0. It is a simple questionnaire that includes a rating scale for each question. The scale ranges from "Poor" to "Excellent". Each rating has a particular value (Poor = 1, Excellent = 4). The participant selects a rating for each question and at the end of the survey a summary is produced on a separate page. The summary changes depending on the participants responses, and it is based the total value of the ratings.

For example, if there are five questions and the participant rated each question as "POOR", then the final score would be 5. Then based on the final score, a summary of the questionnaire is produced on a separate page. The summary will show different text based on the participants score. A score of 5-10 will show paragraph #1, 10-15 shows paragraph #2, etc...

I have looked at the Adobe help files/tutorials and I have been all over the web for advice. So far I have not even been able to find out if a questionnaire is even possible to create.

If you know how to do this, or if you know that it's impossible to do, or if you can at least point me in the right direction to get started or you know of an online tutorial, PLEASE let me know!
