I'm try to make a form for a small doctors office. I would like the form to lookup vaules from a patient registration table and add these vaules to a hospital reservation form. I made a prameter query, and made a form from it and it works fine untill a patient need to make another reservation, the fields from the first form are filled and the form is complete. Which is how the query should work. How can I make it so that when I open the form and give it the prameter only the patient information is filled in. Thanks to anyone who can help!!

We're doing this in Access, I presume?
Sounds to me like you need a form for the patient info, and a subform for the reservation info.
