Need help setting up UG50 with 4COTB on SV8500

I'm trying to get this UG50 programmed and to work on my SV8500. I got the licenses installed for it. I followed the UG50 install manual as best as I could but it's not registering with the phone system. In the past, I've successfully programmed MG-PRI and also the 8 analog line port MC card, so the subject is not totally new.

However, there are some new concept here that I'm not 100% sure about. The UG50 can be in IPG mode, retrofit mode and multi-slot mode. I think I need multi-slot mode. So I settled with that.
But do I need SIP more or Proprietary Protocol mode? For example, I need to use the AMGIL command to associate the MAC address of the 4COTB with the programmed LENs (I went through ARTD, ATRK, MBTK commands), and the MAC address is a virtual MAC address coming from the UG50, but if I put the 4COT4 in SIP mode, then its MAC address on all ports show as 00-00-00-00-00-00. So looks like I HAVE TO use Proprietary mode. I see the virtual MAC addresses then.

Anyways, is there a cheat sheet for this? Or can at least someone shed some light on my above questions? I couldn't get it to register with the system so far.
I have pretty much every protocol handlers programmed in my system: MPH, PHE, PHI, PHC for sure.
 
For Migration Mode in the initial setup leave it as 0:Disable the default, In COT basic set up EQUIP_TYPE change it to Type 2, In AMGIL kind is MG(6COT)

Still no go..
Is it supposed to be SIP mode or Proprietary mode? And what's the registration port?
And do I enter the floating IP of LAN1 (it's a dual system) or the physical IP address?
 
In 1-2-02 the VoIP network set up.
In 1-3-02 put in the LAN 1 float address and port 3456
In 1-4-02 put the LAN 1 float address and port 5060
As stated previously in 4-1-2 EQUIP_TYPE is 2
4-1-03 make sure it is 1 - assuming you are in North America
Everything else should be default. Also important info go to the home screen and click the logout icon this saves the data then log back in and click the restart icon to reboot the UG50. I have installed a few of these and they work great.

Nice it works!
There is one thing I had to do differently:
in AMGIL, KIND had to by MG(6COT)[SIP], instead of just MG(6COT).
