make sure you have v 2.1 of the user software as anything prior doesnt remain set and needs re-enabling everytime phone manager is run.
Apart from that if it still doesnt pop outlook, make sure you can dial from outlook, you have CLIP enabled on the phone line and the number format seems to make a difference in outlook contacts as well.
I found I had to let Outlook decide how the number should be formatted in the "Number" field. It wants to put a +1 in front of the number - LET it (assuming you're in NANPA). Even for my trial here in my lab, I used an analog port off another 403 to pass "203" as the caller ID number, my contact in outlook needed to have "+1 203" as the phone number before it would work.
