Need help diagnosing hardware problem

Apologies for the vague subject, but since I don't know what's wrong, the best I can do is describe the symptoms.

The 11c at a location I am working on is intermittently experiencing card failures.

It began with the sole Analog card's red light turning on, causing all 500 sets to ring fast busy. Re-seating the card has temporarily fixed the problem, but every few hours it seems to happen again.

Additionally, a couple of times my console has completely locked up and I had to INT to get things going again.

After the last time I was told the phones were down, I went on site with a replacement analog card, but this time the red light was on the Digital line card.

Re-seating this got things working again, but at this point I don't know what's going to fail next.

And because I don't know where the problem is, I am unsure how to proceed or what parts to look after replacing, aside from the "call your vendor" route.

As far as error messages, the console went pretty crazy spitting things out, the only one I've noticed recurring enough to write down was IOD041 02

My manuals only reference an IOD0041 n error, not sure if this is the same thing.

Any suggestions are appreciated.
 
IOD041 refers to your TTY 2 (Which tends to be the call logging port). The Nortel message indicates it's..

TTY n is producing incoherent or too much (over a defined threshold) input. The pack has been disabled. Either the device is faulty or it is connected to a very noisy data line. This information is also displayed on the maintenance display. With X11 Release 19 and later, I/O Port auto Lock out Recovery is operating. This message indicates the TTY was left disabled because it had been locked out and re-enabled three times in the last 30 minutes. Action: Manually re-enable the port to bring it back to service. Severity: Critical

Check all of your TTY ports and remove any connections for the time being.

Also if you can, go into LD 22 and print off the history file to check for any other clues.

What software version is it?. It it a Cabinet or Mini version?. Power units can cause wierd problems.

I did find this below.

IF YOU EXPERIENCE IOD 041 ERROR MESSAGES AND/OR SDI PORT DISABLING WHEN USING MCA'S CONNECTED TO AN SDI PORT ON THE SYSTEM MAKE SURE THAT THE MCA ON THE MERIDIAN END IS SET TO HOST MODE NOT TERMINAL MODE. WHEN SET TO TERMINAL MODE THE MCA TRANSMITS ITS MENU INTO THE SDI PORT WHEN IT SEES A CARRIAGE RETURN AND THIS IS TREATED AS DATA OVERLOAD/GARBAGE BY THE MERIDIAN.

another thing that you may need to check is your groud, I've had alot of weird things happen on switches that were not grounded properly.
 
Yep agree with ricoa01 usually random problems moving around is Power/Earth related.
