Need escalation to create a single record

I've created a form to track metrics for our PCR system
implemented using Remedy. I have an escalation that
should create a single record identifying the product
and week. However, I'm getting several thousand records,
one for each PCR.

I can't seem to find a method to automatically create a
single record that I can populate with data. It seems there
has to be a way to do it simply and elegantly. I thought
I'd done it before, but in a different life...

We'er using Remedy v. 3.5 on Solaris. I don't remember what
the database is, we just switched it. Maybe that's part of
the problem, but I hope that's irrelevant.

DOH!

It was a pilot error.

I had misremembered what I had previously done. I was
trying to do in a single escalation what I had previously
done in two.

In two, it works fine...
