NEC Elite 192 dropps all calls

I have an NEC elite 192 with 1 expansion cabinet. We have an occasional problem where CO lines 1-8 (not sure, its possible 1-16) blink red on all the phones and any active calls are dropped. I have 3 CO(8) cards followed by 3 ESI(8) cards in the main cabinet. We have a Winstream Pri-T1 terminated in an Adtran 624 as 12 analog lines. Analog lines 1-12 are terminated on the CO(8) cards as ports 1-5, 9-13, 17-21 in order (as reported by Electra elite IPK SAT program). Occasionally (and more frequently the last 2 weeks) we have had all co lines 1-8 blink on all phones and all active calls dropped. I have not been able to observe the status or heartbeat lights at the time of the issue but its starting to happen more often (intermittently and unpredictable, I would have to sit and watch the channel bank and PBX lights all day until it happens again).

1) Any suggestions on what could cause this? Could this be a second initialization/reset occurring? What should I be looking for?
2) Any reason why my COI(8) cards are only showing up with 5 ports (all 3 of them)?

I didn't configure the system but I also see a note inside the cabinet from a previous tech (another company) labeled slot 7 & 8 bad in the main cabinet so I was wondering if it could be a backplane issue?
 
Since it is only affecting CO lines, I would put a call into Windstream and see if they have any error reports from their router. Possibly power hits or router resets.
Does it recover on it's own with no card reset required?
Two bad slots in a cabinet is never a good sign. Possibly a previous lightning hit. If Windstream cannot find fault on their side, then I would give some consideration to replacing the system rather than putting good money into a system that old.
