Well it looks like one of our 4 port voice mail cards had died. I have tried to find a replacement card online but I don't know the part number and most of my google searches have pages of user guides. I found only one at it was around 2500.00 which seems a little steep for an old system. I found an entire system for sale around that price as well.
We are left with only one working 4 port card and so far I've had no complaints.
Any suggestions?

Are you sure it's an ad64 and not an ad8 as I saw one sold here for Au$250 just recently!

Look at the back of the VM card near the jack. There should be a small white sticker on the card with the dialogic model number of the card. It's easy to miss especially if it's blocked by the PC metal. Typical designations are NE4xx if it's a digital integrated card. The NE stands for NEC integrated. Sometimes you need to open up the PC to determine if it's a PCI card or not. They are still available.

Unfortunately, I live in another state. I just still manage everything remotely. I remoted into the VoiceMail and under device manager it says
Dialogic D/42NE-PCI Card.

Is that the number?

That is the model number of the card you want. Supply that to a dealer and you should be able to get one. You could try eBay but there usually is no guarantee.

Thanks for all your help. I bought a card on ebay for $40.00 worked perfectly.

Happy Holidays everyone!!!
I have an AD40 and am replacing it..I have the correct dialogic boards(D/42 NE-PCI)and the system sees the boards but the ports read
"NO LINE"..I connected the digital ext to the port on the outside pins but it does not see the ext no matter what port I connect the digi sta to...in the system setup I have the vm ports designated

Make sure that you connect the green + red leads to the PBX port and the yellow + black leads to the VM port. You can wire up back to back RJ-11 surface jacks as a tester.
