NEAX 2000 IVS extension is busy but can' t find anyone using it

Using a NEAX 2000 IVS I have this extension that is busy and the phone can not be used on that line, also calls to this ext go directly to voicemail. I had this happen once before on a different ext but it seemed to clear up on its own after a few hours. Any ideas?
 
The ext. is call forward all,that is why it is going to voice mail and the busy may be because it is forwarded to some awkward number.Try the reset code for CFA,ours is #5.
 
If the phone was set to CFA the person could still use the phone. In this case if you pickup the phone gives the sound as if it was in use. Also if you try to use this ext from another phone it also gives the tone as if it where in use. You can't enter any codes. This extension is working fine again (it didn't work for about an hour).

 
You could try using the E8 command see Here
and trace the connection. if not you are going to have to try and see if there are any line appearances for this number and see if that is the problem. If it is a line appearance that would explain it's immediate forward to VM as it is busy (check CFWD busy) maybe try cfwd busy to your No to check.
