Name showing up in from not right.

We have a user which when he sends out an email his name shows up fine in the from area. If he forwards an email his name is not right in the from area. It shows
/o=ou=/cn=/cn= (with all appropriate spots filled in). He is the only one which has this issue. Can someone point me in the right direction?

Sounds like a directory replication issue...
