NAM 4.0 problem

Have a MIC's 4.1 with a 4.0 NAM VM. Periodically when you dial extensions within the switch it will say extension not available. Not sure if it might be a DTMF problem or something with the KSU. Any suggestions are much appreciated, thanks in advance.

P.S. I have tried switching ports multiple times, but problem still persists. you can call in once and get ext. not available, then the next time it might go through just fine, like it isn't recognizing the DTMF??????
