MySQL driver question

I am trying to convert my Access 2000 database to a MySQL database.
My server people told me to install MySQL on my server and setup a username and password, which I have done.

They also told me to download the MySQL driver onto my PC so that I can export my Access 2000 database to my server in the MySQL format.

When I go to setup the MySQL Driver on my machine, I am given a tab with the following questions:
(This is also the tab at START>SETTINGS>CONTROL PANEL>ODBC DRIVERS>MySQL)

1. Windows DNS Name: (I assume I just give it any name)
2. Server: (I assume this will be my server name)
3. MySQL Database Name: (this part I don't understand, if anyone has setup the MySQL driver before and know what goes here would appreciate the info)
4. User: (is this the username I setup with MySQL?)
5. Password: (is this the password for that username?)
6. Port: (I know what to put for this)


Again, I have MySQL running on my server, all I need is to setup this driver corectly so I can convert MS ACCESS databases to MySQL.

If anyone can help, or can point me in the right direction I would very much appreciate it. Thankyou,
