What style phone? Someone could have sent that phone a display message that is causing the light to stay on.

I don't know off hand how to check display messages on a 5410. I usually ran into this issue with 4412/4424 phones. Did you also check to make sure they don't have an extra entry in their source number tab? Maybe they have a hunt group entry in their source number.

Create a shortcode like this and dial it on the phone with the light lit:
SC = *90 ( or what ever you use as a shortcode )
TN = U"; Msgs=0"
Function = Display Msg
