MWI will not light at One extension

System: Definity G3R R011r.03.0.526.5/Intuity Audix 4.1-5
Issue - One extension is not getting MWI.
What's been tried:
1. Removed all messages from mailbox, cleared amw all and left a message. No MWI
2. Removed subscriber from Audix and re-added, left a message. No MWI
3. Removed extension from Definity, Removed subscriber from Audix, Re-added both and left a message. No MWI
Have been checking 'status station xxxx' nothing ever comes up for Message Waiting:
Any ideas out there?
And yes, the Message Lamp Ext. and Audix Name fields are correct.
 
Have you checked ( You have to start with the basics) the station form and make sure the coverage path goes to the hunt group for voicemail?

Next would be to check the "ch auto routing tables"

My audix is rel 5.1, but I'm having the exact same problem. I've done everything that Textron68 has done, in addition to rebooting our Audix system, and no MWI. And yes, I checked to make sure that the lamp setting was set to the same extension and the cover path is set correctly. I checked the MWI settings on tag 2 of the station form to ensure they were the same as everyone else's. I even went so far as to replace the phone, and this has resolved nothing.

I'm at a loss. My main concern is that this problem will begin to creep to other phones, too. I wasn't able to find too much on the forum in the way of help.
 
Mikey - Coverage path is good. no trouble recieving messages, just trouble being notified and only at one extension.
nothing listed in 'change auto-attend-routing routing-table'.
