A button on someones phone was accidently assigned a MWI for an phantom extension that constantly has voicemails left on it. When I try to clear the button, I get a "The line is currently in use." message.

I assume the voicemail box has to be 100% cleared of voicemails before i can remove the MWI button from the phone?

thanks

You assume (somewhat) correctly. The deal is that the MWI indicator has to be "off" before you can delete the button.

I've also seen an instance where if you delete the phantom key appearance first that it will falsely claim that the associated MWI button is in use.

Ergo, the procedure is to:
1) turn off the MWI indicator
2) cancel the key app of the associated like
3) finally cancel the MWI button

the devil is in the details....

Is there a reason the Mailbox is not being cleared regularily?

The Lamp must be off to be changed or deleted.
