MVO400 second incoming call not ringing

I hope someone can help here, I have an mvo400 which has an auto attendant and analogue lines.
first call transferred from Auto attendant rings in and gets answered fine, but the second call only flashes and is not audible.

any advice will help thanks

Flashes on what? Keys?
Where is the AA transferring the call, a user, a group, a CDE?
