multiple xhtml references included on same page

Hi!!!
I have an xhtml form and I require it to be referenced multiple times on one of my html pages. I've tried SSI with an SHTML page and the story goes, it works fine with just one inclusion, but as soon as I include another reference, the page shows blank with error in code for Internet Explorer. I've tried the php way also but same problem. I just need the same form repeated multiple time on a page and can't get it done. Thanks for any help!
 
Trying to repeat a form using simple includes will end up with repeated IDs and form element names.

It will have to be scripted and the element names need to change for each form instance.
