multiple choices in a word template

Im trying to create a template in which the first option is to select a company from a list of 5.

Ive tried using option buttons and check boxes, but they seem to disregard any formatting (also my vb coding talents are VERY limited

How would I go about doing this?

File "test.dot" sent.

I created a form with a drop down text field. I added 5 companies in the selection list (you can add as many as you want) and then protected the document as a FORM (TOOLS -> PROTECT DOCUMENT, select the FORMS radio button) and then saved it as a template document (.dot).

 
had a look at it and it works really well! Only problem is that im not sure whether the end user wanted to be able to select more than one company...
 
...but how did you do the protect function in the first place? Mine is greyed out :(
 
If the user wants to select more than one company set up 2 fields with the same entries.

I don't know why your protect document would be greyed out. Are you working on a networked machine and loading Word from a server?
 
figured it out. i had tried to input a combo box and as soon as that happens, you can no longer protect a document.
Also the user only wants one entry so thats good news.
