multiple choice in a list_item

hi every one,
i need help on creating in some way an multiple choice in a list_item
i thougt maybe to create a block bounded to a table with a non database column
that will be a check box item.
but i don't know how to 'run' no the block records and get the value if
the box item is checked or not an acordinly to act.
