Multi-level TOC Numbering

I have a book that contains multiple chapters. Each chapter has 5-levels of numbers (e.g., 1.2.2.1.1). When I create the TOC for the book, I can't get the multi-level numbering to display. If I turn on autonumbering then I can't get the Chapter numbers to update using the <$chapnum> format.

Does anyone have a clue how to set this up properly?
 
Yes I do. When you select "Add" "Table of Contents" are you including all the tags you want to display?

After the TOC is created you have to go to the TOC then "View" then "Reference Pages" then page 7 or 8 and format the TOC tags, and put in tabs.
