Multi Instance Table Sub Report Getting Duplicates 1

I am probably taking the wrong approach to this and could use some help. I have a table that could have multiple records for each person. The key would be an ID and a Type. I created a formula for each type. If there is a record for that type print out the info from that record if not print out "Would you like to add "type"?" This is a sub report linking to a customer table by ID with one record per person. The problem is if there is more than one record for an individual, it prints out the subreport multiple times. I can't quite figure out the logic to this one.

Any suggestions? Thanks in Advance
 
If you group by the ID you can put the subreport in the group header or footer and it will only run once per customer.

After further checking, I realize that by putting these formulas in the group footer it only gives me the results of one formula and ignores the rest. For example if I have three records for one person with a formula for each type of record in the table it only gives me results for one of the records. The others default to the blurbs as if it did not have this record type for the individual.

Any suggestions? Thanks!
 
OK, don't put the formulas in the group footer. Put the formulas in the detail. You can still leave the subreport in the group header or footer.
