MS Outlook form: setup a command button to send form

Does anybody familiar with building applications with Outlook 2K?

I have 2 questions:
1. I'd like to create a button name "send form". How do I send the form when I click on it? (reason is: when send off the form, if one wants to reply it, he/she can't send the form back, instead the text message which is not good. I need to make that available so one can send the form itself back once they've received and approved the form)

2. How do I get the sender information using the script? For instance, some certain fields, I need to get the value of the field based on the mailbox's name, and emailaddress)
It would be nice to set the value of the sender's info by the mailbox's indentity...
