MS Access Database in Front Page Help!

I am running a PC with MS Windows xP Professional and using MS Front Page 2002 to build and publish a personal web page. I have my own Domain registered and set up.

For the past 3 or 4 months, I have been able to set up a database page on my website using MS Access 2002 and using the "Wizards" in Front Page. Everything has worked perfect up until about 2 weeks ago.

A couple of weeks ago, I had a disk error and ended up formatting my hard drive and re-installing everything. I was updating my webpage database (which is only a Guest Book) and after I published the new page, etc, I get the following error when accessing the page:

Database Results Error
The database connection named 'test' is undefined.

This problem can occur if:
* the connection has been removed from the web
* the file 'global.asa' is missing or contains errors
* the root folder does not have Scripting permissions enabled
* the web is not marked as an Application Root

One or more form fields were empty. You should provide default values for all form fields that are used in the query.

I have tried EVERYTHING and can't get this to work correctly. I have verified the database connection within Front Page. I have been told that I have to "enable" scripting in IIS (which by the way I have never used nor had IIS installed on my machine in the past), I have tried completely rebuilding the web page and nothing seems to work. I keep getting this error.

I have called the web hosting company and they built a "test database" page on my site and it worked fine.

Has anyone ever had this problem? Any help or thoughts will be greatly appreciated.
