Momentum SIP trunks... has anyone used or programmed?

IP500v2 v9.0

customer ordered SIP trunks from Momentum and gave me the following info to use. i just dont know where how to configure this in the IPO?

Momentum gave me the following info to use:

User Domain: mymtm.us
Outbound Proxy - pbx.east.mymtm.us
Line/Registration: 8652217118@mymtm.us (this is test number)
Auth Name: 8652217118
PW: xxxxxxxxxxxx

You would need to create a SIP Line for this and fill out the necessary fields such as the ITSP Domain, Proxy (under Transport), SIP Credentials, modify SIP Advanced, etc.

Should have this knowledge, it's pretty basic.

Usually the other setting can be left at the defaults but search for how do this for other providers which may give some clues.
